Been using Propecia (1 mg finasteride) daily for about 2 years now. Helped with thinning at the crown, or top area of my head. As Scott said, it did not help too much around the front hairline area for me either. The pills should help stop the hair loss, keeping it from getting worse almost within the first week or two. After another few months you may start seeing some new or thicker growth. As for side effects, I have not had any to report, although there are many unwanted ones listed.
Minoxidil (Rogaine) also helps a lot. You can get the regular dropper in liquid form or the new foam stuff. The foam stuff is great, but, if you have a lot of hair it keeps the medicine away from your scalp where it needs to be. Also, get the generic minoxidil, same formula as Rogaine for much cheaper.
I suggest getting yourself a prescription for the Propecia and using a topical minoxidil as well. See how it goes for a few months. That combination should at least keep you from losing any more hair.

Propecia were the tablets offer to me, at 1mg a day.
Yes the side effects, of a 'mr floppy' were a little off putting.
At the moment the price is quite high, £30 a month, but will see if the doctor can prescribe it instead.

Yes, propecia (1mg finasteride) works. Been on it for 10 years now and have more hair than I had when I started. It's great at growing hair on your vertex (top) and anterior (back), but not really the hairline at the temple (front). It's gotten so cheap lately too. I got my doctor to write a prescription for Proscar (5mg finasteride) and split it up into fourths, since you only need 1mg a day. Costs me about $8 a month, far cheaper than any shampoo or other topical solution will cost. I know it's inconvenient, but just go see a doc and get a prescription for it and you'll stop hair loss and slowly start regaining hair.
