BMW offers a manual transmission X5. They are a little hard to come by but it you look deep enough in the car market you should be able to find one.
Jeep Wranglers usually come with manual transmissions. They look like the toughest SUV out there. Plus its not girly.

The Porsche Cayenne has a version that has that option. It's a great truck, I own the GTS version with a 6 speed man. gearbox and have yet to find something I dislike.

I have an '08 Honda Element and I absolutely LOVE it, its offered in a 5 Speed Manual Transmission.

The VW Tiguan is available with a manual, and it's very reasonably-priced. It's a soft-roader, though; I wouldn't go rock-hopping with it.
I agree with DaveD -- Volkswagen Tiguan. You can get a manual while getting almost 30 mpgs, comparable to some hybrid SUVs, which this is not.
Or, if you have the money, the larger Volskwagen Toureg TDi is pretty sweet. I would look into it.
It really depends on how much you have to spend and what you want to do with it. If you're going to offroad - Get a jeep.

If you like to drive a manual... get a sports car.
Toyota Land Cruiser, Land Rover Discovery, Defender, or old Range Rover, Ford Bronco. They all come in standard if you're looking long enough. Any real Sports Utility Vehicle will offer standard as an option. Its just as important for an SUV to be standard as your sports car, as it allows the driver more control. I personally drive a manual BMW 325i, and love it, but have recently been looking into getting a manual Toyota Land Cruiser from the 80's, or a Land Rover. Very well love, and have earned the badges of "Off-Road Vehicles". As Land Rover advertises: "70% of all vehicles EVER built are still in use today". That a pretty bold statement, so take a look. Just stay away from them once they get into the models and years where it was more about luxury then utility. Leather and power window buttons don't like to be in a river under 3 ft of water, even IF you installed a snorkel.

My brother has Nissan Xterra and loves it. It comes with optional 6-speed manual transmission (I believe his is manual, not sure)
A lot of people think its a girly SUV, but his has a custom aftermarket grill guard and looks great.
