I used to frequent a bar in Columbus, OH that always added a splash of Guinness to their Bloody Mary, and I swear it's still the best I've ever had. I don't remember the exact recipe, but I tracked one down that I believe is close:
Tomato or regular V-8 juice
Lea & Perrin's Worcestershire Sauce
Fresh Horseradish
Tobasco
Juice of 2 fresh Lemons
Beau Monde Seasoning
Fresh Ground Pepper
Ketel One or Absolut Peppar Vodka
Splash of Guinness
For an individual bloody mary, I usually do the following in a 14-oz (lowball or rocks) glass.
1 Shot Vodka
1 Tbs Hot Sauce
1 Tbs Lemon or Lime Juice (fresh)
2 Tsp Worcestershire
Then put in ice until glass is half full, and top off with tomato juice.
You can always mix it up with Horseradish, S&P on the rim of the glass, etc. etc. but this basic recipe is the best and easiest for me...
Considering the responses above include a reference to a book without a recipe and a that lacks tomato juice and involves a gross amount of worcestershire... I'll throw this out for consideration. I usually go freehand in a large ice tea glass:
Couple turns fresh coarsely ground pepper
1.5 heaping teaspoons grated horseradish
Shake celery salt
Couple shots of tabasco
Couple squirts of worcestershire
Quarter lime, squeezed and dropped into glass
Quarter lemon, squeezed and dropped into glass
Vodka to taste
Fill with ice, top with V8 (obviously, tomato juice is OK, I happen to like the taste of V8)
Stick in a piece of celery
Garnish with a skewer with a couple olives
Sometimes I put a shake of Old Bay seasoning in it too.

All of the above are great recipes, but I personally add a blob of A1 steak sauce to mine. There's just something about the A1 that make it manlier.
V-8
Absolut Peppar (mix to taste)
Horseradish
tabasco
a few drops of habanero sauce
cracked pepper
lemon juice
Mix V-8 with as much Vodka as you like, add the rest of the ingredients (I like mine kinda spicy so I use a lot of the hot ingredients)
Shake over ice
Pur and garnish with a celery stalk and a dill pickle spear. (the pickle juice adds to the drink too)

I have bloody mary's and white russians flowing through my veins. Everyone has a different perception in my opinion of how a "good bloody mary" tastes like. I don't condone mixed normally, but Demitri’s Classic Bloody Mary Seasoning Mix is absolutely incredible. Add tomato juice and vodka and you're all set. They make other ones if you like them spicier or with more horseradish too. Find the balance between the mix, juice and vodka you like and don't forget that combination. Having this mix in the house is 30 times easier than having all the ingredients.

Check out Nigella Lawson's recipe in her Nigella Bites. Best recipe I've come across, because it doesn't have any gross crap floating in it, like so many Bloody Marys do.
