If you have good water pressure in your house go to lowes or home depot and buy the cheapest 2 dollar head that they make. its usually chrome or white. all plastic.
Remove the head from the female part until all that is left is the little ball piece of the female part that connects to the wall spout. Using the biggest drill bit that will fit down into the female end of the piece, drill out the plastic very slowly until the hole is as big as it can get. Re-assemble and enjoy.
warning...not for environmentalists or water savers...
We have a Waterpik "handheld" showerhead and it's great, better than any I've used at friends' houses or hotels and guests remark that it's good as well.
I like the handheld model because while it's in the bracket it works like a normal fixed showerhead, but you can also take it out and use it to wash something specific, for example if you need to wash sand off your feet without getting your whole body wet.
We have this model:
Waterpik SM-451
It has a couple "massage" settings but we never use them, just the "full body coverage" setting.
Waterpik Medallion 2 Spray Flexible Shower in Chrome Model # CF-203GG
I am tall...I also really like to enjoy the shower experience and all. This one works great because it makes the shower head adjustable taller (or shorter) and it has the right settings and pattern.
I have bought three of these for my various bathrooms over the last couple years and still love them. You can get it at most lowes and home depots. Here is a direct link for home depot:
I like it because it is really adjustable, it has the 2 perfect settings (Waterfall, and high power) and it has good water pressure to get the job done (IMHO). Some others have lots more settings but I've found that you just don't use them. This one has the 2 most used settings. I've tried 2 others in the past but I came back to this one. I keep buying them because I moved so much. Highly recommended by me. :-)
We have the Captain's Quarters from here:

Kinda pricey, but awesome. Most of the time we only use the middle head, but when you need a good blast, having all three going is wonderful.
Speakman gets my vote for the best shower heads - lots of power, infinitely adjustable from hard needles to gentle rainfall. Built to last, and has instructions on how to remove the flow limiter "for cleaning". Check out this model:
I can't help myself: the best answer I can give involves a willing brunette.
The best shower head that I have ever owned is the Fire Hydrant Spa Hand Held from a company called Water Management. It has great pressure and a fantastic flexible hose.
I've gotta say, I can't stand weak water pressure so I know how you feel with the shower head hunt. My favorite so far is this Delta faucet with h20kinetic technology it's relatively cheap and has a switch to go from tame showerhead to skin-resurfacing, soap-removing awesomeness.

Airjet showerhead doubles the pressure while saves almost half of water. it's very impressive!
