I'm looking for a warm place/island to go for a vacation not to far from the U.S. East Coast. Looking for something that doesn't scream "spring break" and something that isn't too "touristy".
Born and raised in Naples, FL. Quality place to vacation. Not touristy or Spring Break-y.
I am fond of the Florida Keys. Avoid Key West, book in advance.
You can fly in on the cheap to Miami, and the route 1 trip over water is something everyone should do once before they die.
The islands between Miami and Key West are not tourist-y, have their own local flavor and are affordable.

One of the best places is the Outer Banks in North Carolina. While a bit cold for the beach in the true winter (like pretty much the whole East Coast), it is a great place to visit that fits your criteria of not Spring Breaky or touristy at all.
The farther south you go on the banks the more relaxing it is with almost a feel like nobody is there, but still plenty to do. I stayed at this B&B called the “Inn On Pamlico Sound”: http://www.innonpamlicosound.com/index.php and it was truly the best. Steve the owner took care of us like no other, the breakfast was incredible and different every day, and while we didn’t have the dinner because we wanted to go out, I heard nothing but good things. The place has a movie theatre, kayaks, pools, bikes, free drinks and more…truly a great B&B. Next time I go back I will stay there for sure! Outer Banks and this B&B are the best.
